I agree that my answers may be placed in a database. I agree that my scores can be placed into the survey data bank, as long as my identity is not revealed. I agree that, if I choose to reveal the results of my survey to anyone, I am fully responsible for any damage that may occur to me or anyone else because I revealed that information. I agree that, if I choose to reveal the results of my survey to anyone, and damage results to me or anyone else, I will not bring any legal action against Donna Daisy, Ph.D..

Donna Daisy, Ph.D. IS NOT “COUNSELING” OR “THERAPY”

Donna Daisy, Ph.D. is an interactive, educational source for testing and information. You provide information on behaviors you observe in yourself and or your partner. We provide responses tailored to the input you give. Our responses tell you possible negative or positive implications of the behaviors you observe. Our responses offer information about healthy and unhealthy relationship patterns. Our responses are based on research and professional experience.

WE DO NOT ADVISE YOU TO LEAVE, OR STAY IN, ANY RELATIONSHIP. WE DO GIVE YOU OUR EDUCATED OBSERVATIONS ABOUT WHERE CERTAIN BEHAVIORS OFTEN LEAD. No decision about leaving or staying in a relationship should be based upon test results.

WE DO NOT INTEND TO ESTABLISH A THERAPIST/PATIENT OR COUNSELOR/CLIENT RELATIONSHIP. Donna Daisy, Ph.D.. professionals could not offer therapy or personal counseling over the internet. Among other things, extensive personal interaction is required for therapy or counseling. The information about you is for educational and research purposes only. It helps researchers classify your responses to the survey questions about your relationship and partner. THE QUESTIONS SEEKING INFORMATION ABOUT YOU ARE OPTIONAL. I understand and agree that Donna Daisy, Ph.D. is not providing me with advice, opinion or recommendation about whether I should stay in my relationship or not. I do not now claim, and will not claim in the future, that I have any therapist/patient, counselor/client or similar relationship with Donna Daisy, Ph.D.. I covenant, meaning “I promise,” that I will not sue Donna Daisy, Ph.D. for any claims relating to my use of this survey. I understand and agree that Donna Daisy, Ph.D. has no liability to me, and I agree to waive any legal claim, now or in the future, against Donna Daisy, Ph.D..

VALIDATION OF TESTS

Donna Daisy, Ph.D. does occasionally uses test results for statistical purposes, but only in an aggregated and anonymous form. Occasionally, we gather test data in order to validate our tests. Each visitor has the choice of submitting his/her information for validation purposes. Even in this instance, Donna Daisy, Ph.D. never uses individual data (name, place of residence, etc.), but uses aggregated (collective) databases for purposes of test validation. Otherwise, no records of scored tests and quizzes are kept.

CONSENT TO TRANSFER

This Web Site is operated in the United States.
